LOWLINC Walk and Talk Group

Members and volunteers meets at the Community Center every Friday 10:30 - 11:30 A.M. Walkers can choose between walking in the gym and, weather permitting, walking on the Sweetbriar path. Caregiver Discussion Group

If you are a caregiver, it can feel overwhelming but you are not alone. If you could benefit from meeting with others to share challenges and successes of caregiving and learn more about support services and resources available, please join us. The group is open to LOWLINC members, LOW residents and the Locust Grove community. We meet every other month (generally on a Tuesday at 1:00-2:30 PM. at the LOW Community Center). Please contact LOWLINC (855-569-5462) to register and confirm meeting dates and times.

 

 

In-Home Support Guide


Developed by the LOWLINC Health & Wellness ad-hoc Commitee, this guide is intended to help older adults who may need or benefit from in-home assistance at some point in time. The guide describes different categories of in-home care providers to assist LOWLINC members and their families, as well as other members of our community, understand the differences among various levels of care and identify appropriate level(s) to fit their needs.

Health Matters Toolkit

Developed by LOWLINC with a grant from the Culpeper Wellness Foundation, and based on materials from the National Institutes of Health National Institute on Aging.

Provides guidance on taking an active role in your healthcare, including preparing for a visit to your doctor or other healthcare provider and maintaining records to keep track of your health concerns, medicines and vital signs.

Medical Note-taking

LOWLINC has trained volunteers as Medical Note-takers to accompany members to a medical appointment for the purpose of helping a member document the details of the appointment. The notes can serve as a reference for the member and family members. For example, the Medical Note-taker can take notes on the conversation between you and your healthcare professional, note your vitals and record new medication orders and information about any forthcoming diagnostic tests. The Medical Note-taker also helps the member prepare for the visit, reviewing questions to be asked and then clarifying information that is recorded. The goal is to help a member stay in charge of their health care. Click here to learn how our Medical Notetaking Service can help you during healthcare appointments.
